Criminal Justice Council Forms for Use by Subgrantees

This is a listing of Criminal Justice Council forms which are most often used by Council subgrantees. All forms are in Microsoft Word format. Form	Description
Subgrant Application	This form is used by approved subgrantees to apply for funds. This document becomes the contract under which the subgrant is administered.
Quarterly Progress Report	This form is used for the quarterly reports which are due within 20 days of the end of each quarter.
Final Fiscal Report	This report is to be completed within 45 days of the end of a subgrant.
Request for Funds	This form is used by active subgrantees to draw down funds.
Subgrant Adjustment Request	This form is used by active subgrantees to request an amendment to the project budget.
